What Is Periodontics?
Periodontics is the branch of dentistry focused on the structures that support your teeth — the gums, the periodontal ligament, the cementum, and the alveolar bone.
A periodontist is not a general dentist. Periodontists complete three additional years of postgraduate MDS training beyond their BDS degree. That specialist training covers the diagnosis and management of all gum disease, implant placement, and mucogingival surgery.
Periodontal disease begins with a bacterial film (plaque) on the tooth surface and under the gum margin. When plaque is not cleared away, it triggers an inflammatory response. Over time, that inflammation destroys the bone and connective tissue anchoring your teeth. Most patients feel no pain until the later stages — which is why regular assessment is so important.
Periodontal Conditions We Treat

Gingivitis — Stage 0–1 Gum Disease
Gingivitis is the earliest and fully reversible stage of gum disease. Signs include red, swollen gums that bleed when you brush. At this point, no bone loss has occurred.
Treatment is non-surgical: professional debridement, oral hygiene instruction, and adjunctive laser disinfection. Left untreated, gingivitis progresses to periodontitis.

Chronic Periodontitis — Stages 2–3
This is the most common presentation in adult patients. Probing depths exceed 4mm, clinical attachment loss has occurred, and bone loss is visible on X-ray. Most patients are unaware until they attend for assessment.
Treatment at Dental Solutions Clinic follows an evidence-based, stepwise protocol: non-surgical debridement first, laser-assisted bacterial decontamination second, and surgical intervention only when residual pockets persist.

Aggressive / Stage 4 Periodontitis
Advanced periodontitis with significant bone destruction, tooth mobility, and the risk of multiple tooth loss. This is where laser periodontal therapy (LANAP and WPT) delivers outcomes that conventional surgery frequently cannot. CBCT-guided bone assessment is essential for treatment planning at this stage.

Gum Recession — Mucogingival Defects
Root surface exposure due to gum recession can lead to sensitivity, aesthetic concerns, and an increased risk of root caries. Depending on the recession classification (Cairo RT1, RT2, or RT3), treatment involves root coverage procedures, including connective tissue grafts, laterally positioned flaps, or laser-assisted recession correction.

Peri-implantitis
Inflammatory disease affecting the tissues surrounding a dental implant — the periodontal equivalent of periodontitis around a natural tooth. Treatment requires mechanical debridement of the implant surface, laser decontamination, and, in some cases, bone regenerative procedures.

LANAP — Laser-Assisted New Attachment Procedure
LANAP is the global gold standard laser protocol for Stage 3–4 periodontitis. It uses the Nd-YAG wavelength of the Fotona LightWalker to target diseased tissue and bacteria inside the periodontal pocket — while leaving healthy tissue intact.

WPT — Wavelength Optimised Periodontal Therapy
WPT combines the Er-YAG and Nd-YAG wavelengths of the Fotona LightWalker in sequence. Together, they remove calculus, biofilm, and diseased tissue more thoroughly than either wavelength alone. WPT is particularly effective in Stage 3 periodontitis and in patients whose previous conventional surgery did not fully resolve the disease.

Conventional Flap Surgery

Is gum disease reversible?
Gingivitis — the earliest stage — is fully reversible with professional debridement and improved home care. Periodontitis is different: the bone loss it causes is permanent. But the disease itself can be stopped. With the right treatment, we can halt the destruction, preserve the remaining bone, and in some cases — particularly with LANAP — stimulate new bone growth. The prognosis for individual teeth depends on disease severity, your general health, and how consistently you attend maintenance appointments.
What is the difference between LANAP and WPT, and which is right for me?
Both treatments are laser-based procedures for periodontal disease, generally offering better results than traditional surgery in most cases of moderate-to-severe periodontitis. They typically result in less trauma, reduced post-operative pain, and quicker healing. LANAP is a patented protocol that uses only the Nd-YAG wavelength and has the strongest evidence supporting new bone growth. WPT employs both Er-YAG and Nd-YAG lasers in sequence, enabling removal of calculus and hard deposits that Nd-YAG alone may not effectively eliminate. Dr. Balasubramanya will recommend the most suitable protocol based on your specific situation, including pocket depths, bone structure, calculus load, and your previous treatment history.
Can laser gum treatment really save teeth that other dentists have recommended for extraction?
In some cases, yes. This is not a universal guarantee — teeth with less than 25% remaining bone support, severe mobility, or furcation involvement may have an irreversibly poor prognosis regardless of treatment. However, patients routinely arrive at Dental Solutions Clinic having been told their teeth cannot be saved, undergo LANAP or WPT, and, at the 6-month CBCT review, show stabilised or improved bone levels and reduced tooth mobility. We document every case with serial radiographs and clinical photographs, and we are transparent about the prognosis from the outset.
How many appointments does periodontal treatment require?
Non-surgical periodontal therapy (scaling and root planing) typically requires 2–4 appointments, depending on the number of teeth affected. LANAP and WPT procedures are typically performed in 2 appointments — one for the upper arch and one for the lower arch — each lasting approximately 60–90 minutes. Surgical procedures require additional appointments for surgery and suture removal. All patients enter a periodontal maintenance programme following active treatment, with recall intervals of 3–6 months.
Does dental insurance cover periodontal treatment in India?
Coverage varies by policy. Most dental insurance plans cover basic scaling and root planing. Surgical procedures — including laser periodontal treatment — may require pre-authorisation.
